As of July 1, 2008, we are required to have written consent in order to release vaccination records to non-veterinarian facilities, such as kennels, groomers, and trainers. If you use any of these services, please fill out a Medical Release Waiver. You can do this by stopping by our clinic, downloading the form (see Boarding/Grooming Page) and faxing it to us.
